Upon enrolling, you must commit to 12 months. The membership fee will be scheduled for automatic draft on the same day every month. 

Memberships fees are due by automatic withdrawal on the date of enrollment each month

Members will not pay any initiation fee at time of sign up

All members must provide a 30-day written cancellation notice to avoid any further membership fees.

Membership privileges will be terminated at the end of the month following members 30- day written notice.

Card information must be given at time of sign up. Members agree that if that information changes, member will give Spa 3:16 new card information to be able to continue membership

Members understand that membership privileges will be immediately revoked in the event that Spa 3:16 is not able to obtain your monthly payment

Members understand that free monthly treatments cannot be carried over to another month or transferred to another person, even in the event that member is not able to take advantage of the services that month.

Additionally, free treatments and membership discounts can only be utilized by the member. Member privileges are not transferable.

Members percentage off discounts cannot be combined with any other discounts or specials.
